POSITION DESCRIPTION

 

Title:                                       Transition Coordinator

 

Reports to:                             Director of Transition and Empower U Kent ISD

 

Terms of Employment:           200 Days

                                               Professional Contract- Grade 8                     

           

BROAD STATEMENT OF RESPONSIBILITIES:

 

The Transition Coordinator will support countywide special education transition services for students aged 14-26 under the supervision of the Director of Transition. The coordinator will support transition programming and services offered at Empower U, KCTC, and KCTC Career Focus, including transition curriculum and high-quality instruction, data-driven program improvement, compliance monitoring, professional development, and working with families and community agencies. The Transition Coordinator will assist the Director of Transition with the training, compliance, and coordination of countywide special education transition services in collaboration with member district administrators.

 

MINIMUM QUALIFICATIONS:

 

  1. Valid Michigan professional certification in an area related to Special Education is required.
  2. Master's degree in Special Education or related field, preferred.
  3. Three years of successful experience in special education.
